Swami Satyanangananda Saraswati decodes akotiah as “pressing” and so does Lakshmanjoo.

Dmitri Semenov (good name for a Tantric researcher), graciously parsed this at my request and writes:
“I analyzed the compound ... lehanaamanthanaako.tai.h -- as instr.plural of the compound, lehanaa-manthanaa-ko.ta

lehanaa here is a feminine action noun from verb lih (lick)
manthanaa is a feminine action noun from verb math (rub)
ko.ta is a masc. noun meaning "curvature"

It is a bit of a stretch since lehanaa and manthanaa are not in the M.-W. But such forms are allowed. So, I translated the whole compound as:

"by curvatures [of her body], by [continuous] licking and [continuous] rubbing"
